I'm really surprised Neat Image didn't do a better job - I've gotten really great results with it. Two things you could try...select the background and do a really light gaussian blur - sometimes that will cut way down on the noise that is detected. Also, you could try the blur tool - make it fairly big and put the opacity at 50% and do all around the areas that have noise in them. This tool is really great at helping to pull away the noise but it does leave the area with a blur so make sure not to get it where you don't want it.
One last thing to check is how you are saving the image. If you use Photoshop CS3 or Elements to do post processing you might want to skip using the "save for web" option as that tends to give me worse results than if I use the plain "save as" option. If that's not the case, then one last thing is to make sure you have the neat image profile for your particular camera...that can sometimes help. Feel free to PM me if you want to discuss it further.
