Kinesis has one. Also consider getting one for a 600, that way if you change it will fit and you can have the camera mounted for short trips.

I know the F4.5 is a lot skinnier but cases for a f4 have been talked about here:

I would buy a non photographic company photo backback that can fit a 500/4.5. Photo companies usually gouge photographers X10 over non photographic gear.
Try mountainsmith, dakine, burton, apollo, any non photographic company that makes a photo backpack will give you a real value for your dollar.

I needed a bag for my 600 and couldn't find what I wanted in any of the online photography resources. I was thinking of making one. Requirements;
1. very compact and lightweight - primary use is not to carry the lens on trips but to fold and put in my luggage. I carry my 600 in a backpack from Moose Peterson (I'm a Canon person but his big bag is perfect - it carrys my 600 and fits in the overhead of an RJ)
2. for use laying it in the backseat of the car
3. must fit with the lens hood on
4. zips all the way open so it's easy and quick to take out and zip up
Solution - $40 - it's a padded bag made for an archery bow. They came in 2 sizes, I got the larger one. I saw it when I was browsing around a sporting goods store. Also a yoga mat will fold in half and fit perfectly. Some extra padding and something to lay on if needed - although I wouldn't put it back in the bag after being on the ground without washing it off.And there's a zippered pocket on the side which was made to carry arrows - my Gitzo fits perfectly. Room for the Wimberly inside with the lens as well - I put a small piece of foam between it and the lens.
