Intense stare, soft light with subdued but effective color palette. Shooting angle adds a lot. I think the angled neck adds to the sense of 'tension' in the image, which fits well with the stare.

I agree with Randy, but I'll only add that I find the image a little too yellow - try adding some blue in Selective Color and see if you like that.
